prompted by the invalid ascii85 tests failing. act_ks_value wraps its result
in an HParseResult and parse failures get swallowed. not yet sure what the
correct course of action is, but i wonder why act_ks_value is an action at all
and not a continuation on an h_bind.

also move the include to where it is needed

fixes segfault when /Columns = 0
